Mental Health Care Power of Attorney
The Durable Mental Health Care Power of Attorney legally appoints the person(s) you have chosen to make future mental health care decisions for you if you become incapable of making those decisions for yourself.
Your representative will have the following responsibilities:
Receiving information regarding mental health treatment that is proposed and consent to disclosure of treatment if needed.
Consent to the administration of any medications recommending by the treating physician.
Admitting the person to a structured treatment setting with 24 hour-a-day supervision at a level one behavioral health facility.
Typically, the persons acting as representatives are those named in your medical power of attorney for health care.
